Vision & Goals’ supported coordination is here to support you and your family or carers on your NDIS journey. Our disability support coordination services are person-centred and individually tailored. We will work closely with you and your family or support people to make sure we understand you and your support needs.
What is Support Coordination?
Support coordination is a capacity building NDIS support which works with you to:

Connect you with the best NDIS providers, community, and other government services

Build your confidence and skills to use and coordinate your supports.
What is Support Coordination?
There are three levels of support coordination that can be included in your plan:

Support Connection: Builds your ability to connect with informal, community and funded supports so that you can get the most out of your plan and pursue your goals.

Coordination of Supports: Builds the skills you need to understand and use your plan. A support coordinator works with you to make sure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be part of your community.

Specialist Support Coordination: A higher level of support coordination for people with more complex needs and situations who need specialist support. A specialist Support Coordinator will assist you to manage changes and help ensure consistency.
What Does a Disability Support Coordinator Do?
A disability support coordinator or support care coordinator works with you to make sure the right mix of supports are used in your plan.
A support coordinator’s goal is to support you to increase your capacity to maintain relationships, to manage service delivery tasks, to live more independently and to be part of your community.
What is specialist support coordination?
Specialist support coordination is one of the three levels of support coordination that can be included in your NDIS plan. It is the highest level of support coordination. It is for people whose situations are more complex and who need specialist support.

Understand your accommodation and home support needs and options, including supported independent living, in-home disability support services, short-term accommodation (respite), medium-term accommodation, and specialist disability accommodation.

NDIS funding information (including eligibility criteria)

Your NDIS funding is based on what the NDIS considers reasonable and necessary.
To Be Considered Reasonable and Necessary, A Support or Service:

Must be related to a participant’s disability

Must not include day-to-day living costs not related to your disability support needs, like groceries

Should represent value for money

Must be likely to be effective and work for the participant, and
-
Should consider support given to you by other government services, your family, carers, networks and the community.
Where reasonable and necessary, support coordination will be included in your plan’s capacity-building budget. This is a fixed amount for a support care coordinator to help you use your plan.
If your plan describes the level of support coordination funded, you can only purchase this level of support coordination.
If your plan does not describe the level of support coordination funded, you can choose to purchase the level of support coordination that suits your needs.
